This form is used for all events, whether on campus or off campus, whether using facilities or not. 

You will need to submit this request at least 3 weeks prior to your event for approval or we will be unable to service your request.

Guest Services Department
Bethel does not supply food, drinks, dispensers, dishes, paperware, or cutlery. The event organizer is responsible for providing these items if needed (staff needs to submit a PO for any such items).

  • Please make sure that you coordinate with all of the departments you have requested on this form at least 3 days prior to your event to make sure you have everything needed.
